1. PSA Birth Certificate - Original and Photocopy

1. Passing of Interview and Qualifying Exam
2. Original Form 138-A or Original Report Card (Kindergarten)
3. Form 137-A (Kindergarten permanent Record)
4. Medical Certificate

Documents needed upon enrollment
1. Grade 6 Report Card (Original and Photocopy)
2. PSA / NSO - authenticated Birth Certificate (Photocopy only)
3. Certificate of Good Moral Character
Documents that may be submitted on or before the opening of the classes
4.Two(2) pieces of 1x1 picture in white background
5. 2 long brown envelopes and 1 short envelope
6. Recommendation Form (available in the Guidance Office to be filled out by previous class adviser or principal)
7. Medical Certificate issued by the physician
8. Student's Information sheet (available in the Guidance Office)
9. Student Health Record (available in the School Clinic)
10. Form 137 (SF10) upon school's request
Requirements for SENIOR HIGH SCHOOL
Documents needed upon enrollment
1. Grade 10 Report Card (Original and Photocopy)

4. ESC Certification (for Grade 10 completers from private schools)
